Our Legacy
We are an 87-bed, skilled post acute care facility located at the corridor of
Pleasant Hill, Martinez and Concord. We have been providing our community
with outstanding nursing, rehabilitation services and extended care for over
two decades. Whether for short-term post hospitalization care, complex
nursing care, rehabilitation therapy services, or long-term skilled nursing care,
our staff of trained professionals are dedicated to providing a professional,
nurturing and respectful environment. Along with post joint replacement
therapy, we specialize in stroke recovery and complex wound care.
